PersonalDr waiting list privacy notice

This notice covers the waiting list form on personaldr.ai. PersonalDr is run by Azaris Healthcare LLC, 19667 Turnberry Way, 16J, Aventura, FL 33180. Questions: azaris@azarishealthcare.com.
What we collect. Your name and email address, your confirmation that you meet the eligibility questions, and the date and time you joined. If you check the text-message box: your mobile number and a record of your text consent (the date and time, the consent wording, and the page address). We do not keep your internet protocol address with your waiting list details. Please do not send us health information.
Why. To email you about PersonalDr's launch and memberships. If you check the text box, we may also text you once we start sending texts. Joining is free, is not a membership, and does not make you a patient.
Who can see it. The waiting list is stored in our Google account (Google Sheets and Gmail) and may also be copied to our Wix contact list. Our website is hosted by Wix. These companies store the data for us. We do not sell your information, share it for advertising, or give it to anyone else unless the law requires it.
Website basics. Like most websites, our host may record technical details such as your internet protocol address and browser type to run and protect the site. We do not use advertising or tracking tools on this page, and we do not respond to browser "Do Not Track" signals because we do not track you across other websites.
Adults only. The waiting list is for adults 18 and older. If we learn that someone under 18 joined, we delete their details.
How long we keep it. Until you ask us to remove it.
Your choices. To see, correct or delete your details, or to be removed from the list, email azaris@azarishealthcare.com. Every email we send has an unsubscribe link.
Changes. If we change this notice, we will post the new version here with a new effective date, and email people on the list if the change is important.
This notice covers only the waiting list. When PersonalDr opens, members will receive a full privacy policy and a Notice of Privacy Practices.
